Please provide a one-page summary write-up regarding the need for the donation at least three to four months prior to your request’s due date. Please provide the following information:

	Date of fundraiser.
	Nature of the request.
	Name of person or organization who receives the money and/or donation?
	Name of person or organization who specifically will benefit from the money and/or donation?
	What is the purpose of the money and/or donation?
	What is the percentage of money and/or donation used for administration purposes?
	Your 9-digit federal tax ID number. This is either an organization’s EIN (##-#######) for a school or charity, or an individual’s social security number (###-##-####).

Please include any and all relevant information that we should consider in reviewing your request. We cannot give to all causes; however, we do what we can to give back to key elements of our community. If we are able to help, we will contact you.
